Continuous Glucose Monitoring (CGM) - FreeStyle Libre 2 Reader

FreeStyle Libre 2 CGM sensors are small, unobtrusive, and easily applied to the back of the upper arm with an applicator. The small, flexible, tip that is inserted just under the skin continuously measures and stores glucose readings when worn on your body and transmits the readings once connected to the reader or a smartphone.

• Sensor Memory Storage: 8 hours
• Glucose Reading Frequency: 1 minute
• Glucose readings can be viewed on the FreeStyle Libre2 reader, or on a smartphone with the FreeStyle Libre2 app
• Sensors and reader covered by Medicare, PPO, and some Medicaid plans for adults and children with diabetes, ages 4 years and older, with a Physician's written order.
